Nurses and Directors lie They will tell you anything just to get you out of your face worst facility Beavercreek Post acute is way better and they have Great customer service skills .Centerville Post Acute starting from check in desk is very blah and don’t have any customers service skills
Find another facility staff is very rude
First of all, I would give this place a zero star rating if it weren’t for the (literally) one or two nurses that were kind and helpful. I am appalled and angry at how this place treated my family member! Problem 1: In the short amount of time they were there, me and other family members visited often and were not made to feel welcome. Additionally they handed out the password to us, and others visiting family, to get in and out of this unit of the building, because there was never anyone at the front desk. Quite the safety concern there! Problem 2: they were not giving my family member their medication at the proper schedule, which impacted them mentally and physically. Problem 3: about half of the time I arrived, my family member had medicine all over themselves so they clearly didn’t take it all. And no one cleaned it off of them. Problem number 4: most of the time the entire place smelled like pee and poop because residents had gone to the bathroom on things or themselves and they hadn’t cleaned it up yet. One chair with poop on it sat outside in the “courtyard” for at least two days that I saw. While my family member was at another facility for two and a half weeks, their room was never cleaned. Additionally, upon multiple visits, there were pee stains on the floor that hadn’t been cleaned for days. Problem 5: the lack of communication between the facility and our family, and even the other facilities working with my family member, was absolutely ridiculous! They rarely answered or returned calls and often didn’t tell us about things like them falling until way after it happened. I understand the facility is understaffed, but that is no excuse for the conditions and what felt like an overall lack of concern from the administrative staff. VERY disappointed!
I would not recommend this facility to those looking for a nursing home for their loved one. I understand that many places are understaffed, and I do believe the nurses did the best with the available resources. However, the facility was not clean and there was a very strong smell of urine and feces, especially in the memory care unit. The facility just isn’t taken care of well. The are also security concerns. We were given the codes to the memory care unit, but that is not allowed in other facilities. I believe this ties into the fact that they are understaffed. We also were never asked to sign in when arriving or sign out when leaving. Another item to note is that the sign near the elevators for employee of the month is from 2024. I’m sure the employee is great, but 2024 is a bit dated. These might seem like minor issues, but when the little things are missed it makes me wonder what else is being forgotten. My biggest concern was the lack of communication from the administrative staff. This was an issue to the family and there were even times the nursing staff was not made aware of pertinent information. Overall, I believe my family member is worse off than when they arrived. Post Acute Centerville is not equipped to handle the volume of residents in their care and the administration needs to do a serious overhaul of their processes.
My 18 year old daughter took my uncle out driving without even showing I’d he has dementia and thought he was with a nurse she didn’t even sign the book at the front desk then 4 weeks prior we noticed bites all over him they said they were treating it so 4 weeks later we see these bites were still on him we withdrew him and took him ourselves to a doctor which said either scabies or bed bugs they gave us some medicine to put on him and it’s gone completely.so Brittany the nurse in charge had the audacity to call adult protective services on us for removing him do not take your loved ones here if you really care about the treatment they will receive..Do better post acute centerville.btw post acute of Beaver Creek did great while he was there no complaints with them
If it was possible for negative stars I would do that. The treatment my mother received was so poor. One aid verbally abused her. Her phone charger was stolen from her. There is zero security as to who can walk in and out on the weekends. They lie about therapy time given. They don’t make sure meds are actually taken. They don’t monitor patients at all leaving family to call for emergency care. And so many other things. Don’t put your loved one here.

Don’t put your loved one in here. My family’s experience was not good. If I could, I would give them zero stars. Didn’t honor or respect a Do Not Resucitate order; disturbing comments made to me on the morning of my mother’s death, by a very rude and disrespectful higher up person; On the day of my mother’s death, the new top administrator called me, and after speaking to him, said he’d check into my concerns. I told him I didn’t want something similar to happen to somebody else’s loved one. It is over a month since my mother died, and I haven’t even gotten a call saying they are still researching the issues and need more time. Nothing. I called the top administrator, on his cell phone, this morning, and left a message to return my call. The day is now over. Still nothing. It is shameful. YOU CAN’T WORK IN A PLACE OF CARE, IF YOU DON’T CARE. SO BEWARE WHEN MAKING DECISIONS FOR YOUR LOVED ONE. BECAUSE NOW, I HAVE TO LIVE THE REST OF MY LIFE WITH THE DECISION MY LOVED ONE COUNTED ON ME TO MAKE.

My LO stayed a week in November 2025 at this facility’s Memory/Respite Care. I really liked the Facility. My Mom was welcomed with open arms, and made to feel comfortable. As even the process in itself is confusing and daunting, all my questions were answered with patience and understanding. From Brandi in Admissions to the nurses and aides put my mind at ease. I will be calling on them in the future. Thank you for caring for my Mom!

My husband is in memory care, but we are all over the building every day. We find friendly, caring, competent staff on the units and in the specialty areas. My husband benefits from PT, OT, Speech & Swallowing therapy, and mental health therapy, all of which are very good about communicating with me. Special events that are planned for the residents are beyond anything I’ve ever seen, and I have six years of experience as an RN in long-term care, in addition to having both of our parents In similar conditions as their lives came to a close. The teamwork that I find on the memory unit is wonderful to see, and the way that I see management chipping in to help in so many ways tells me that the leadership there is key to the many good things that I find. I am well aware that no place is perfect, because no person is perfect…. I certainly am not! But I see a an underlying dimension there at CPA that is not often found…. That is that I truly believe the leadership and staff have a genuine love for the people they care for. That factor is enabling me to accept that my husband of almost 60 years is truly cared for, and I can be at peace with him there.

Do not send your love one here! Picked up my relative and found his foot full of infection. Drove straight to the hospital and er doctors were shocked at the amount of neglect. Went back to retrieve his items and there was only one nurse and one aid in the whole facility.

I would like to take this opportunity to express my sincere gratitude to the entire team at Centerville Post Acute for the outstanding care and compassion they provided to my mother and our family. After my mother was hospitalized and her previous facility declined to readmit her, our family was faced with the difficult task of finding another long-term care facility. I visited several locations but did not feel confident about any of them. On my way home one day, I decided to stop by Centerville Post Acute without an appointment, and that decision turned out to be one of the best choices we could have made. Upon arrival, I was warmly greeted by Brandi from admissions. She welcomed me with a smile, gave me a tour, and patiently answered all of my questions. Her kindness and professionalism immediately gave me a sense of comfort and trust. As I toured the facility, every staff member I passed acknowledged me with a greeting something I had not experienced elsewhere. That genuine warmth set Centerville Post Acute apart. Once my mother was admitted, the care she received was far beyond what we expected. The nurses not only tended to our mother’s needs but also checked in on our family to ensure we were comfortable and informed. Two nursing aides Kay and Devyn showed remarkable compassion and dedication, and my mother connected with them as though they were members of our own family. I would also like to commend the wound care physician Sylas, whose attentiveness and kindness during the final stages of my mother’s life made a lasting impression. Additionally, Lori was an invaluable source of guidance in helping me navigate the Medicaid process. Her knowledge and support made a challenging experience far more manageable. We would also like to extend our sincere thanks to Garrett, the Rehab Unit Manager, for his professionalism and for taking the time to thoroughly answer our questions regarding our mother’s medical care. Our family has only positive things to say about Centerville Post Acute. The entire staff demonstrated genuine care, respect, and professionalism at every step. We are deeply grateful for the comfort and dignity they provided to my mother and for the compassion they extended to our family. After a five week hospital stay, I was transferred to Centerville Post Acute for another five weeks for Physical and Occupational therapy. My husband initially came to check everything out before I was discharged from the hospital and he was treated with such kindness and felt that it would be a good fit for me. When he signed off I knew that I would be in good hands. I have had a wonderful experience throughout the whole process, from admissions through discharge. Everyone has taken such good care of me, went well above and beyond the minimum, and truly listened to any questions or concerns that I had. I was never made to feel like a burden and if I ever need inpatient therapy again in the future I would definitely come back here. I would also recommend the facility to family and friends. Thank you Centerville Post Acute staff for everything!

I had a family member come through here and the whole experience was incredible from top to bottom. From the friendly and knowledgeable staff, to the facility itself, we were more the satisfied. I spent 100 days in Centerville Post Acute and was taken care of by several employees. Nurses, therapists, doctors, just to name a few. Sometimes I really didn’t want to go to therapy due to pain from my injuries and was told they would go easy on me. Then I recalled “no pain no gain”. I pushed on and know they certainly were the best therapist as I am now living alone in my home since July 20th. I was only 1 of many to be attended to and found it very rewarding to receive a smile from the individual coming to my room to draw blood, give me pills or a bath, bring my food tray and cut the meat. No matter how short handed they would be I never felt neglected and I would highly recommend Centerville Post Acute as a place for family and friends to spend their time for recovery or therapy if and when needed. I actually do my outpatient physical therapy here and I absolutely love my therapy team. So accommodating and the goals they have set for me are responsible. The whole staff is encouraging and everyone from Reba at the front desk to the CNA’s always have a smile on their faces. I highly recommend outpatient physical therapy here as well as for recovery from surgery as this was the location I was at last year for my recovery from my left leg being amputated. I can not say enough more about the quality of care and compassion from the whole staff 10 ⭐

We are honored to post a review for Centerville Post Acute. They received with open arms, our dear friend who became a quadriplegic through an accident. After I contacted numerous other facilities that didn’t even want to consider taking in Paul, this team was more than willing to do all they could to get our friend Paul into their wonderful establishment. This was an out of state transfer and they helped us with all the necessary state paperwork (which was a nightmare for me) and handled it with ease. The care and love that Paul has received from the staff at Centerville Post Acute has been second to none. He’s receiving medical, dental and eye care along with physical therapy. Being a quadriplegic, Paul has to depend on good people to handle all his daily needs and they have with such dignity and care. They have treated him like family and we are so thrilled he’s back in the Dayton area where his family and friends are able to come visit him. I highly recommend this facility to take excellent care of your loved ones!!

The first thing I noticed was how nice, clean and organized it was. The rooms are nice and big and can accommodate all my guests when I have many at one time. The people here are so nice, helpful and polite! I would recommend this facility for rehab to my family and all my friends as well! I will only come back here if I need rehab in the future!

This place has been very good to me! The staff is attentive, they listen to me and they care. They are down to earth and friendly with great senses of humor!! I enjoy giving everyone a hard time and the smiles on their faces! Overall, this has been a great place to rehab and live.

I have been living here over 6 months and I’m extremely happy with my care and the staff members are all amazing!!! I highly recommend this place for you and your family members as I have been other places and this is the BEST!!! The other residents are also friendly and happy too!! I will never go anywhere again! This is like home!!!
